ELIGIBILITY REQUIREMENTS
- The candidate you are submitting through this application must be available to film during July/August 2024 (final dates TBD).
- You must be at least 18 years of age and a legal resident of the United States to submit this application. If you are submitting this application to nominate a child, you must be a parent or legal guardian of the nominated child and your nominated child must be between the ages of 14 and 17.
- If you are selected as a potential participant, you will be required to execute waivers and release agreements required by Producer and Network or any of their licensees, successors or assigns.
- Potential participants may be required, and must agree, to undergo background checks and/or physical, medical and psychological examinations and additional testing. Participants must be willing to submit medical information to Producer and submit to a medical examination, psychological examination, and a background check.
- As the program will include on-camera conversations with mental health professionals, a HIPPA release form will be required.
